Can You Appeal Health Insurance Claim Denial?

I am trying to have a breat reduction and the my health insurance company had denied it. What can I do?
I do have back problems: neck problems, and always have sore muscles. The Dr. said in the initial visit that it would probably be denied: He was planinning on an appeal with a panel of Phycisians.


Answer: Christy, breast reduction is considered a cosmetic surgery and such a claim is not allowed as the terms and conditions have specified clearly. However, if the doctor's report can clearly prove that it is needed on medical reasons; then your claim should be approved. Do check with your insurance agent for more details.
 
 

Why Does A Health Insurance Company Request An Independent Peer Review Company To Review A Patient's Appeal?

I am in the middle of a second level appeal with my health insurance company. They are now requesting I sign and send them a release of my medical records to have an outside peer review company investigate my medical records in order to make a decision on my case. Why are they doing this? What should I do? I do not feel comfortable doing this but I also feel they would deny the appeal if I decided not to do this? Anyone know about this or what rights I have regarding this situation?

Answer: The outside peer review is an unbiased board, meaning they will just look at, and make their decision on, the facts of the case. This is part of your contract with the insurance company and if you refuse to cooperate the appeal will get dropped. If the appeal is dropped you have no further recourse.

What Is The Best Way To Appeal A Health Insurance Denial For Coverage?

Husbands insurance company wont cover me. Stating "dont meet eligibilty requirements". Cite previous medical condition and that company waived HIPPA. Told me to reapply in 6 months to a year! What do I put on the letter or form?


Answer: Well, it's going to depend on the medical condition and what you mean by "waived HIPAA."

Are you coming off of a group policy with less than a 63-day lapse in coverage? Then, as long as the group policy was a HIPAA qualified policy, they can't waive HIPAA under Federal law. However, it's all in the details.

Are you working with an agent? If so, call and ask for guidance. You CAN sometimes win appeals. I once spent six and half months helping someone win his appeal (and they covered him back to the original application date.) It can be done, but it will depend to a great degree on the specific reason they've declined you and whether you have an agent

Should I File A Health Insurance Appeal?

I am self employed and I work full time for one Company as a consultant. While leaving their office one day I twisted my ankle and fell. I saw my doctor and assumed it would be covered by my health insurance. The claim was denied. I found out after the fact that the insurance company had sent a letter to my husband asking about the injury to my foot. Without even asking me or letting me know he filled out the form telling them it was a job related injury. So the claim was denied because it was deemed a claim for Workman's comp. I believe this was not a job related injury. I just happened to twist my ankle walking to my car, does it make any difference where I was at the time? It had NOTHING to


Answer: File an appeal. This would not be considered a "job related injury" in most cases.

The fact that you didn't fill out the form may not be an issue; that depends upon if your husband is the primary policy holder.

The company you consult for will not be responsible for the bill unless they were negligent and caused your injury.

Answer: I have. I've appealed several, for my family, successfully. It's all about pointing out how the insurance company was wrong, or didn't have enough information to make a judgement.
