Answer: 1,000,000 personal liability plus coverage on all your professional equipment for replacement value. Most homeowners policies will not cover your equipment if it's used in your business. A lot of public places require proof of 1 mil liability insurance for professional shoots. Malpractice is also good to protect you from things such as losing wedding photos because of a bad card.

Answer: I hate to be rude but it's hard to talk to people who don't have a clue here.. Do you drive a car yet..? If you do, you MUST know what a insurance agent is. Even if any of us here WERE insurance agents, it would be imposable to give quotes not know the list of equipment you have, how much deductible your willing to pay, the value of your insurance and such...
Do you own a home..? Most home insurance policy's cover loss or theft of items when away from the house. Check with your parents and have them talk with their insurance agent.
It will be hard to have anything replaced while over seas. You would most likely have to wait till you got home for much to happen.

Answer: I've only shot freelance and all insurance, of any kind, is my responsibility. That covers health, unemployment, business interruption, equipment insurance, liability & worker comp. That's pretty much the story for freelancers.
If you shoot for an agency and are on staff, some of that is covered, but I don't know about health (possible, but what coverage?). I don't know about the chain outfits that do school photos, or portraits in malls, etc. They may have nominal coverage under a group policy. Each business will handle it their own way.
Several local operations that do photos of tourists at various sites do have some sort of medical coverage available
