Can A Healthcare Provider Bill Medicare A Different CPT Code Than What They Bill The Commerical Insurance?

I have seen providers bill Medicare A9270 so they guarantee that Medicare will deny, so they will get more money by having member's secondary commercial insurance pay as primary. Is this legal?

Answer: Billing for something other than what was actually performed is fraud. The situation you describe is clearly fraudulent because they are doing it with the intention of getting more money by having the secondary insurance pay as primary. I would report this to the secondary insurance.

Medically necessary cosmetic procedures are sometimes covered by medical insurance

Answer: Just having the codes will not be sufficient information to determine whether any procedure will be covered by insurance.

Carriers will cover different procedures. Each carrier may also offer different types of insurance coverage so not all plans from the same carrier will cover the same procedures.

It's probably best to work with your dermatologist to determine whether any procedure will be covered.

I am in the market for excellent liability insurance as a cpt. I am not necessarily looking for the cheapest but the most sufficient available. Thanks in advance.

Answer: Well, you're going to have a hard time getting it, period!! You'll need to contact a local, independent agent who is licensed to sell insurance in your state, fill out an application complete with professional resume, and see if he can get more than one or two quotes, so you can compare coverages and costs.

I'd expect it to cost a minimum of $2500.

Answer: Yes, you do, otherwise the insurer's computer won't know what type of service you've had done. The computer then compares the code with the terms of your policy and determines what, if anything, to pay (this is a simplified version of the process).

Participating health care providers should send a claim to the insurer for you; however, if you've seen a non-participating provider, you'll need to send the claim yourself, unless the provider's office staff volunteers to do this for you. If you have to send the bill yourself, some providers will simply print a "CMS 1500" form for you to mail.
